    Yes, I am a Human Resources Officer from a Singapore Company. A Birth Certificate was given to me with Registration No. 2019-616 which is not valid. Our company need to submit for claims
    of Maternity Leave for our staff who delivered her child in LUCENA UNITED DOCTORS HOSPITAL & MEDICAL on the 14 Sept 2018 and a registery of certificate of Child Birth was given to us and it is not valid.
    REGISTER ADDRESS OF CHILD BIRTH : EDITHA A.REGODON,RN,MPH. CITY CIVIL REGISTRAR OFFICER ON JAN 18, 2019.

    Kindly look into the above matter and please update.
