   We are humbly requesting from your good office an original copy of the Medical Certificate and Official Receipts for your patient, MR. NELSON CATALAN admitted to your hospital last December 15, 2014. In order to process the third party bodily injury claim of DRA. APRIL P. ROMULO to MAPFRE Insular, we needed to submit the said documents since these are her requirements for reimbursement for all of her expenses.
